RN M^r Will: Co{l|e|}lles Colles of Parke berrye of 58 troubled sick this i6 yeres & hath had the Counsell of the most exte phisitions troubled w^th a windy cause as he sayth cōming from his backe by a great heate & obstruction from my liver. also w^th a bad stomacke elevating winde into my hed. w^ch causeth gydines or streming in my head w^ch cometh as he thinketh by the ascending vp of bad & melancholy humours & w^th a shaking in my left hand mightely as it were the palsye but always very hotte. nowe of late w^th the brevity of wind in my stomacke & chest & lately w^th a mūmnes {sic} & tinckling in my hands. lips & sydes & face. this 15 of may. die ♂ h. 4. 45. p m. 1599.

RN Rich Kerby Miller of Newport 60 \56/ yeres old. long & grevously sick {filiu}s  patre \{servus}  Domino/ cū euꝯ {sic} consensu it hath held this 4 yeres by fits but this quarter of this yere held him sorely & keepeth home some days his bed in his fit & then abroad. a payne in his hart w^th a sighing black coloured very fat of his face but w^th this sicknes shrunk in his bodye & legs. M^r yarrow for him novemb. {sic} 1 Decēb. die ♀ h. 10. 15. 1598¹

RN Robart West of Haversham for a payne in his loynes & hips & right leg & toes ever since Sunday last at evening at 4 a clocke & hath had a winde cholicke ever since he was a batcheler & his wife also ever sinc a meaden hath bene troubled w^th a gnawing in her body. 56 yeres old. came frō Sherington. RN Will: Seere of haversham. 53 yeres old sitteth vp by the fier, very stiffe & hardly to {ground} onc in 24 houres complayneth of his body & lims & legs most in on of his sides all over his left syde ever sinc March last: first amisse not able to goe over the Dore threshall. sinc Alhalloutide. sent his wife to me Decēb. 21. die ♃. h. 12. 45. 1598. on of his legs the left wonderfully swelled. a very diseased body & hath had a greate cough.¹

RN Iohn Persivall of 56 yeres \of Buckingham./ of 56. complayneth of his head & hart & stomack & would fayne have some ease. Sept. 7. 40. {sic} & hath bene sick in this disease for this 6 weekes. & this 3 yeares & maketh his stomack swell 1598. die ♃ h. 7. 40. ant m.

RN Wull {sic}: Fisher of hanslop of 51 y. his water hie coloured & very red. his sicknes vexeth his him in every te & keepeth his bed. lyeth in his chest & reereth bloode. 22 of may die ♂ h. 2. {sic} 1. 50. p m. 1599

M^r Knifton of Lowton 55 yeres old sick this 3 weekes {illeg} sunday last first in his stomack w^th an extreeme cold w^c tooke a way his stomack ache in the head neck & legs sweateth div^rse times & after troubled w^th an itch as exceeded greatly onc in two or 3 days to grounde. nev^r vomiteth. the 3 of Ian. die ☿ h. 9. 15. 1598. {sic} 1599. bad head greefe taken {in} lawe.

RN Williā Miller Smithe of Brodwell mutch payne in his Chest & stomack vomiteth long ropes of fle{me} in 24 houres sinc Christmas. especially when he eateth meate. came to me this 5 of Ian die ♀ h. 12. 45|9|. 15{99}

RN Richard Langly of Newport 50 yeres old. short winded ever sinc Christmas can scars tak his breath. cougheth mutch red w^th little fleume. came for him selfe this 30 of March die ♀ h. 2. p m. 1599. ague fallen to his thigh & knee & ham. butter annoynted helpeth his aches best.

RN Iohn fleete of Singleborowe ab 54. for his speach & the hoarsnes therof. when he is thorowe hot. his spittle hot & salt & sower. some times in his Chest & hed & minde w^th crosses. Apr. 21 die ♄ h. 1. 30. p m. p m. {sic} 1599. w^th losse of frends & breakes of covenantꝭ w^th his wifes frinds greeve not.

RN Williā Fisher of hanslop of 50 yeres old. his moth^r died on tuesday morninge Alce Fish^r of 60. yeres old. he cōplayneth of his loynes & hed. may the 17 {sic}. die {illeg} ♀ {sic} h. 9. ant m. canot make water & is costive. 1599.
